Exhibitor Registration

When you join to become an Exhibitor at the next conference you are provided with multiple services and products to choose from.


Scientific Program:

$2000.00 will allow you:

• 2 representatives from your company one (1) six foot table during the pelvic surgery (lecture) course on Friday and Saturday to promote your product.
• Advertisement with your company logo listed on the SPRS corporate sponsors page and monthly email newsletter for one year.
• 1 post conference attendee mailing list in excel format.
• Continental breakfast, lunch and break snacks on Friday and Saturday.
• $1000.00 for each additional representative.


Cadaver Program (Separate Fee):

$2000.00 will allow you:

• 2 representatives from your company to attend the hands-on cadaver program to promote your product. (Transportation to and from lab is not included.)
• Advertisement with your company logo listed on the SPRS corporate sponsors page and monthly email newsletter for one year.
• 1 post conference attendee mailing list in excel format.
• Continental breakfast and boxed lunch on Thursday.
• $1000.00 for each additional representative.


NOTE: Any Unrestricted Educational Grant received prior to August 1, 2010 for $5000.00 or more will allow you all of the items listed above with a maximum of four representatives from your company. Faculty Sponsorships and Lunch Sponsorships do not apply.

Faculty Sponsor

Sponsorship will pay for faculty honorarium, travel expenses and hotel room. To request a specific faculty member please submit your request to Sarah Daniels by July 1, 2010. Due to ACCME regulations, this item will be considered a grant and a Letter of Agreement must be signed before August 18, 2010. $5000.00 per faculty member.


 

Lunch Sponsor:

Sponsorship will be used to purchase one sit-down lunch for all attendees, faculty and exhibitor representatives during Scientific Program. Only two lunches available, please submit your request to Sarah Daniels by August 1, 2010. Due to ACCME regulations, this item will be considered a grant and a Letter of Agreement must be signed before August 18, 2010. $5000.00 per lunch.

This exhibitor contract is for exhibit space at the Nineteenth International Pelvic Reconstructive and Vaginal Surgery Conference on August 19-21, 2010. This form will become a binding contract upon written acceptance from the Nineteenth International Pelvic Reconstructive and Vaginal Surgery Conference.


• Exhibitor fee is $2,000.00 per table for exhibitor displays.

• The Exhibitor Rules and Regulations by The Eighteenth International Pelvic Reconstructive and Vaginal Surgery Conference are attached to this contract. By signing below you and your represented company agree to adhere to the rules and regulations set forth by SPRS, Inc.

• Space will be forfeited and no exhibitor will be set up if the contract and fee is not received prior to July 1, 2010.
